How To Make A Claim

If you have sustained an Injury or have been diagnosed with an illness that you consider is a claim under your Policy,
then here’s what you must do:

1. Obtain a claim form by:

A. Downloading it from above;
B. Contacting Marsh on 1300 766 104; or
C. Contacting AFA Claims Department on 02 9259 8213

2. Complete the form

3. Forward your claim form to AFA
